What we do is have the ball defender under the basket just as you described. His job is to defend the basket and yell "ball!" when the ball is thrown in. If it is near a defender he yells the defenders name. I tell the other players to play "butt to the ball" and stay between their man and the basket and watch the eyes of who they are guarding.
